(2018) On 7 December 2017, a serious accident occurred on the jack-up rig Maersk Interceptor, which was working for AkerBP on the Tambar field in the North Sea. The accident occurred in connection with the lifting and installation of a raw water pump. Four people participated directly in the work. A wire rope sling was used in the lift. The strap smoked and the pump fell. A power cable, which was attached to the pump, was dragged along in the fall, hitting two of the people who were nearby. One fell overboard and died. The other was seriously injured. The Norwegain Petroleum Safety Authority writes in their report that the direct cause of the accident was that the lifting strap broke However, the underlying causes of the accident were multiple and complex and can be related to weaknesses in design and their follow-up, maintenance of lifting equipment, training, failure to identify risks on several levels as well as planning and work practices.
